On these little step down transformers, the secondary is 24VAC and has an isolated winding that is no reference to ground neutral. Primary is  L-1 120VAC and a neutral ground.  The trick to making the buzzer give you the 2000VAC between the iron frame and earth ground is in the fact that when contacts break the the energy is in a float condition.  The reason I got that initial shock was the fact that I was working on a ladder and had my arm resting on the metal wall stud cap plate and holding the iron frame of the buzzer when it was energized.  If you are not grounded, you can hold the buzzer and not get shocked. From the simple schematic you see that the single winding around the soft iron armature is grounded at one end to the metal.  This works just like the Modle T ignition box except it has a HV secondary winding to distributor and plugs.
